The completed medical questionnaire that was mailed or available on line to you as well as a list of all of your current medications, past surgeries and any other current medical information.
We accept all insurance plans but you are responsible to verify with your insurance company if we are in your network. Auto accidents and Workman Compensation require pre-authorization before you can be seen. Make sure to bring insurance cards and financial information or referral form if needed.
- Consulting with the Doctor
Please prepare a list of questions ahead of time to discuss with the physician. Your visit will be more productive the better prepared you are. Privacy will always be preserved but some sensitive questions may be necessary for your diagnosis and care.
- What if Surgery is Needed ?
The surgical scheduler will contact the hospital and make all necessary arrangements for your surgery. Pre-authorization will be obtained from your insurance company and pre-operative instructions regarding food and medications will be explained to you by the staff.
Upon discharge from the hospital the nurses will instruct you to contact our office and schedule your post-operative appointment, usually within two weeks of leaving the hospital.
If you have any emergency you may call the office during normal work hours, or the main office number which will put you in touch with our 24hr answering service. |
